There is a mouth puppet that can be made from a single sheet of paper with 
at the minimum 5 folds.

This can be taught en masse I have done it - but of course not to 40,000 
people at once....but have no doubt it can be done.

Language and clarity of instruction is important.......

You can find it in Sheri Lewis's book of paper puppets.

There is also a one move sort of a fold sort of a squeeze that will take a 
card and turn it into a mouth puppet ...business card, playing card even a 
3x5 card.
But I have found that really simple move much harder to teach then the 5 
fold one.

1 ream of paper is 500 sheets = 500 puppets....

Recycled is green! :)
