Here's two more books:

Saturday Morning TV, Gary H. Grossman, Arlington House, Inc. *1966, an
entire chapter on early TV puppetry and many other references throughout
the book. Great pictures, too.

Children's Television: The First Thirty-Five Years, 1946-1981. Part II:
Live, Film and Tape Series, George W. Woolery, The Scarecrow Press, Inc.
*1985. A listing with a description and short history of  practically
everything that was on TV.  Look up puppets, puppeteers, and puppet
companies in the index.

Then, of course, there's always Bil Baird's The Art of the Puppet.

I stumbled on this paperback once, and I was glad I bought it. I have
never 
seen another copy!
It covers kids tv from 1947-1973
Quite specific- only kids shows on the  3 major networks are listed NBC
CBS 
ABC
no PBS, cable, or syndicated shows and no local shows
It gives the date of first and last broadcast for each show, cast list
and 
photos
There are about 45 shows with puppets that are listed
Lots of great info about cartoons too, for those who are interested

Kids TV: the First 25 Years by Stuart Fischer 1983 Facts on File  ISBN
