Yikes.

This is a tricky repair.
Foamcore is weak. which is why I never use it for strutcutral stuff.
But since you're stuck with it at the moment, let's see what can be done.


Carefully peel up a good piece of the fabric from the foam (if possible), 
then make the repairs using Weldbond and coffee filters, torn in tiny 
pieces. I'd dilute the weldbond until it's easier to paint on, but still 
tacky. I'd add about 4 layers to give it strenght.
You can speed-dry the touch up with a hair dryer or a heat gun at low temp 
settings.
Then carefully re-apply the fabric, using pure weldbond, but in a very light 
application. It shouldnt be white, bcauxe that would mean too much glue, 
which would seep through the fabric, or leak away from the seam. The one 
problem iwth Wledbond is that it dries glossy.

Then it should be a paint touch up.  It can sometimes be a tricky step, but 
it can be done subtly enough to not show up on stage. Or it can be made 
obvious, as if the prop had been repaired, giving it a kind of "lived" 
quality.  If it doesn't work, you can still patch it up by making the fix as 
part of the design.
For example, if it's a dog with patches of different colors, add a patch 
where the touch up is.
Why not add a fake bandage, if it's a character?
