I got my copy by setting up a Google alert. Whenever the book was mentioned
on the net, I was alerted. It took about 3 or 4 months of patience, but I
managed to find a listing for it on Abebooks. 

My suggestion is to do the same, and continue looking for it at Ray Da
Silva's site as well. 

There is also a thread at Puppetsandstuff.com on the book (probably from
last year or so), which discusses the possibility of a reprint; someone
actually tried to contact the publisher's about it, though I don't recall
what the reply was.
