It's not the most perfect survey, but I tried to strike a balance between the usual demographic questions and one about activist and anarchist stuff. Sorry if it is a bit too Americancentric. Pick the two languages that you are most fluent in. The survey originally had a matrix of choices that allowed for numerous picks, but I discovered that it didn't work in the way I coded the survey. I realize that the choices don't capture many non-US magazines, but that's mainly because I don't know about all of the ones out there.
