It sounds like you might have some oil in your mold.  Neoprene will not
set-up where there is oil on the mold.  I believe the oil stops the
absorption of the water in the neoprene into the mold or something like
that.

I sculpt in oil based clay, so I always clean out my molds with very hot
water and dish detergent.  I usually don't have a problem with holes or thin
spots.  The neoprene from Chicago Latex seems to have less of a problem with
oil spots than the stuff I got from the California supplier.  I've yet to
have holes show up with the Chicago Latex stuff.
