  Not quite clear on the question or why it makes a difference.  Usually
you will be able to work the puppet rod in either hand, thus the hand rod
will sometimes be beside you and sometimes in the "opposite" hand.   If
this is not the case I would think you would want to work the hand that
will give you the greatest range of motion.  Sorry if this is too vague of
an answer.
  Also, it does not take two hands to work two rod puppet hands.  So
holding the rod of the puppet, be it beside you or overhead, should have
little bearing on the opperation of the puppet's hands.  Then, I ask why
the "dead hand"?
